Family Outreach

The goal of the SOAR program is to assist persons who are experiencing homelessness or near homelessness and have a mental illness and or co-occurring substance use disorder. The SOAR advocate and the participant strive for the participant to become eligible for SSI or SSDI and the corresponding insurance.

SOAR can be utilized for:

  • Gathering needed historical information: education, military, employment, medical records
  • Collecting current medical and employment records
  • Advocacy on behalf of the participant
  • Some financial assistance may be available

Eligibility Criteria:

  • Be in a homeless or near homeless situation
  • Have a verifiable mental illness and or co/occurring substance use disorder
  • Desire to make positive life changes
  • Willingness to participate in regular meetings, phone conferences ect with SOAR advocate

Required Documentation:

  • Based on case by case situation(s)
